What is Hospice Care?
Hospice care is specialized medical care for people with a life expectancy of six months or
less. Hospice is a service that focuses on comfort and quality of life when the illness can no
longer be cured and a cure is no longer desired. Our interdisciplinary team is dedicated
to addressing our patients’ physical, emotional, and spiritual needs while providing full
support for family members and caregivers.

Home-Based Services
We provide our services wherever our patients’ homes are: private homes, nursing homes, and assisted living facilities—so the patients can remain in their familiar environment with loved ones.
Bereavement support
We support families throughout the illness and continue supporting them for over a year after the loss of a loved one.
24/7 support
Our hospice team is available day and night for any concerns you or your caregivers may have.
Who is eligible for hospice care?
Hospice care is available for individuals with a terminal illness and a prognosis of six months or less. Eligibility typically requires confirmation from two physicians that the patient’s condition is beyond curative treatment, with a focus on comfort and quality of life.
Common diagnoses for hospice patients include:
Cancer
Heart disease
Dementia, including Alzheimer’s disease
Chronic kidney disease
Cirrhosis
ALS (Lou Gehrig’s disease)
Parkinson’s disease
Lung disease
Stroke
When to Consider Hospice
Starting hospice care early can significantly enhance comfort and quality of life.
Consider hospice if:
Despite medical treatment, there has been a significant
decline in physical or cognitive health. This may be marked by
more than the pain or symptoms, weight loss, extreme fatigue,
shortness of breath, or weakness.
The goal is to live more comfortably and avoid treatments
that are no longer effective.
The patient is in the end stage of Alzheimer's or dementia.
How to Get Hospice Care

confirm that the patient meets medical eligibility requirements.
What's Not Included in Hospice Care?
Hospice care doesn't include:
Treatments to cure the terminal illness
Room in a nursing home or hospice facility.
Emergency room care arranged by the hospice team.
Prescription drugs not related to the terminal diagnosis.

Special Considerations for Hospice Care
Discharge and Revocation
Patients may be discharged if their health condition has improved or if they elect to attempt curative treatments. Once they become eligible again, they can return through re-enrollment.
Caregiving in a Private Home
Hospice care at home usually requires a family caregiver or another person to provide caregiving services.

Patients with advanced heart disease can avail hospice care with symptom management, medication management, nutritional support, and emotional and spiritual support.
Lung Disease
Hospice Care Supports patients with advanced lung diseases, including Breathing treatments and support Management of Symptoms Emotional Support Nutritional Counseling
Stroke
Hospice care manages severe stroke symptoms, and provides emotional and spiritual support.
Hospice Care for Cancer
Cancer patients experience many symptoms and emotional challenges. Our services in hospice care offer pain control and symptom management, coupled with emotional support for cancer doctors and all their specialists.
Hospice Care for Parkinson's Disease
Unique care for those with Parkinson’s includes symptom management, physical therapy, and emotional support, among other things.
End Stage Renal Disease Hospice Care
Managing symptoms, managing medications, providing nutritional support, and providing emotional and spiritual support to a patient with end-stage renal disease.
Hospice Care for Alzheimer's Disease and Dementia
Hospice care offers comfort for Alzheimer’s disease and dementia patients with personalized plans, integrated care, and family support during the final months.
Amyotrophic lateral sclerosis (ALS) Hospice Care
ALS patients will require aggressive symptom management, assistive devices, emotional support, and coordinated care with neurologists and other providers/specialists.
How is Hospice Care Paid For?
Most hospice patients are covered by Medicare, Medicaid, or private insurance.

Medicare and Medicaid including all aspects of hospice care and services.
Private health insurance plans depend on the specific coverage.
Military insurance through Tricare offers comprehensive hospice benefits.
The Veterans Health Administration provides hospice services for veterans.
We also offer Private Payment (Self-Pay) for the uninsured.
